The forms are: 1097, 1098, 1099, 3921, 3922, 5498, or W-2G.

2. How to fill out Form 1096?

Form 1096

Enter the Business details Filers name, address, name of the person to contact, email address, phone number, and Fax number

  • Box 1: Enter your employer ID number ​(EIN)
  • Box 2: Enter your Social Security Number (SSN) if you don't have an EIN.
